Choice Unlimited Terms and Conditions

The Choice Unlimited Plan is a direct dialed outbound long distance service offered to single line residential Customers from switched access lines. Service is offered on a flat rate basis, twenty-four hours per day, seven days per week. Customers must dial 1 plus the area code, if applicable, and the terminating number to complete a call utilizing this service. The Choice Unlimited Plan provides the Customer with unlimited minutes of interexchange, Intrastate and Interstate long distance usage for a flat rate monthly charge (certain restrictions, as outlined below, do apply). This plan does not provide call detail information on the Customer's Monthly bill.

Customer Eligibility Criteria
New and existing Customers are eligible for this service if they meet the following requirements:

1. Customers must subscribe to Guadalupe Valley Long Distance.
   
2. Customers must presubscribe to Guadalupe Valley Long Distance for both intraLATA and interLATA long distance service.
   
3. This plan cannot be used for any use inconsistent with residential service.
   
4. This plan is not available to Customers with a multi-line account or an account that bills to another number or is the recipient of charges billed from another number unless the Customer establishes separate billing accounts for each line.
   
5. Customer lines associated with educational institutions, (colleges, universities, etc.) are not eligible for this plan.
   
6. Unlimited plan usage does not include calls for business purposes, multi-party conference calls, calls to 900 numbers, directory assistance, calling card, operator services, third party charges, international calling and toll free calling services. Unlimited Long Distance Plans may not be used in conjunction with the following: auto dialers, long distance Internet access, call center applications including but not limited to autodialers, PBX trunks, ground start line or trunks, ISDN services, foreign exchange services, public telephone services, public access smart-pay phones, analog to digital conversion digital PBX services, WATS services, PBX/PABX/EABX services, non-square electronic key telephone systems, hybrid key telephone systems, predictive calling/dialing systems, automatic outbound dialing systems, any type of automatic call distribution system, or the functional equivalent of any such system listed above.
   
7. This plan is not available for resale.
   
8. If the Company determines that usage is not consistent with typical Residential Customer usage, the Customer may be subject to an additional fee or offered an alternative plan at the Company's sole discretion. For the purposes of this plan, typical residential usage is presumed to be total usage that does not exceed 5,000 minutes per month of interstate and/or intrastate service per account.
   
9. In order to be eligible for this plan, the Company must be able to verify that the Customer meets these eligibility requirements. Customers who no longer meet these eligibility requirements will not be eligible for this plan and will be contacted by the Company and offered an alternative plan.
   
10. The Company reserves the right to exclude certain terminating telephone numbers from this plan. Calls to these numbers will be billed at 10 cents per minute.
    
11. If the Company determines that calls are made that are not consistent with residential service or calls are made that do not comply with the stated terms and conditions, the Company reserves the right to change the long distance plan from Choice Unlimited to a per-minute long distance plan.
